AMBOY – Children who live in the Amboy School District and will be 5 years old on or prior to Sept. 1, 2014 may pre-register for Kindergarten on Monday, April 14 from 8:30 a.m.-6:30 p.m. at Central School.
To register parents must also bring their child’s certified birth certificate (not a hospital copy) and proof of residency in the district.
All parents are required to provide evidence of current residency within the boundaries of the school district.
Acceptable examples of proof of residency include:
• A driver’s license or other state issued identification such as a current voter registration card or current public aid card.
• A current utility bill (gas, water), cable bill, bank statement and/or credit card bill with address within district.
If you are a homeowner you will need a current tax bill with the property owner’s name and address or current mortgage papers.
If you are a renter you will need a signed and dated lease indicating the parent, legal guardian or legal custodian’s name and proof of last month’s payment, e.g. cancelled check or receipts. Lease must indicate length of lease. Additional paperwork may be required if there is no lease or the lease is not in the name of the parent or legal guardian.
If you are a mobile home park resident you will need a manager verification (available online) and proof of last month’s payment, e.g. canceled check or receipts.
If you are residing with a family member and all forms of legal residency are in their names, the district resident must complete property owner verification (available online). It will also be necessary for the district resident to provide the above items as proof of residency.
Proof of residence is required, regardless of past attendance in the district.
Parents who pre-register on Monday, April 14 will not need to pay for fees at this time. Fees will be paid at the district registration to be held on Thursday, July 31, 2014.
There will be drawings held for those who pre-register on April 14. Prizes include a 2014-15 yearbook, a class shirt or spirit wear shirt, and paid skating in PE.
After registering, you will sign up for a kindergarten screening time on May 13 or May 15. You will have a brief tour of the school as well.
All incoming kindergarten students must also show evidence of a physical examination, vision exam, and an updated immunization record prior to the first day of student attendance in August. Those forms will be provided at registration.
